Khan Academy: Click on grade level and all standards from the year are covered. There are videos and online practice questions as well as an assessment at the end.
IXL - Click on Learning at the top and then the appropriate grade level. All math standards are covered. This is my favorite website to use and it is user friendly.

Virtual Nerd common core videos for instruction: all math standards can be found here: This website has all grade levels and contains short videos to teach your student the standard by specific categories. This website is one of my favorites to use.

Online coordinate planes practice
Practice with coordinate planes and ordered pairs. Only use first two games. The other games are not at a 5th grade level
Khan Academy: Geometry Unit: This link contains videos and practice questions and a unit test when students feel confident to take assessment. I recommend students practice several times before trying the assessment.
Class K12: Awesome Math website with virtual questions. This website will read aloud all the questions to your student. The students can have it read to them as many times as they wish. The link I am attaching is for fifth grade. All math standards are covered here. Students and parents can choose which standard they wish to practice. This website has all grade levels. If you want to practice skills at a lower grade level just choose the appropriate grade level.
